Baptist Home Health Care by BAYADA is seeking a compassionate, dedicated Full Time Registered Nurse to join our team serving Eastern Jacksonville, FL, on a per-visit basis. The RN will provide in-home care to adult and geriatric clients, following established care plans and ensuring quality, compassionate service.

Responsibilities include medication administration, wound care, and routine assessments. Candidates must have an FL RN license, strong interpersonal and computer skills, and be willing

Qualifications

• Active RN license in FL (current).
• Minimum 1 year of recent clinical (medical/surgical) experience.
• Home care experience preferred but not required.
• Graduate from an accredited nursing program with transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Must be able to travel to cases as assigned.
• Florida Background Screening Clearinghouse compliance.

• Follow a designated care plan based on patient needs.
• Make home visits to clients in designated territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to supervisor.
• Follow up on and document doctors' orders.

• Document observations and interventions using a tablet.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in FL.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
