Job summary

Baptist Home Health Care by BAYADA is seeking a compassionate, full-time Start of Care/Oasis Admissions Registered Nurse to join our Jacksonville, FL team. This role serves adult and geriatric clients on a per-visit basis across Duval, Baker, Clay and Nassau Counties.

Oasis home health experience strongly preferred. BAYADA emphasizes dignity, independence, and delivering high-quality home health care with compassionate, reliable service.

Qualifications

• A current Florida RN license.
• Prior home health or Oasis/Start of Care experience preferred.
• Ability to travel within designated territories.

Responsibilities

• Follow a designated care plan per patient needs.
• Make home visits in Jacksonville and surrounding counties.
• Administer medications, wound care, treatments and procedures.
• Monitor patient conditions and report changes to supervisor.
• Document physicians' orders and client assessments.
• Coordinate case management and care services.
• Document observations and interventions accurately in the EMR.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in FL.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
