Qualifications

• Active RN license in the state of practice.

• Knowledge of hospice philosophy, end-of-life care standards, and regulatory requirements.
• Proficiency with EMR/EHR documentation and timely charting.
• Excellent communication with patients, families, and teams.
• Ability to work independently and manage visits.
