The Avenue at Strongsville, a post-hospital rehabilitation and skilled nursing facility, seeks an RN to provide direct patient care, assess needs, develop care plans, and collaborate with an interdisciplinary team to support resident independence.

Night shift opportunities are available for full-time or part-time status (6p-6:30a). The family-owned facility emphasizes a warm, hotel-like environment with all-private suites and opportunities for professional growth.

Qualifications

• Active RN license in Ohio.
• Basic Life Support (BLS) certification required.
• Minimum 1 year experience in geriatric or skilled nursing setting preferred.

Responsibilities

• Provide high-quality patient care per facility standards.
• Perform initial and ongoing assessments and develop care plans.
• Implement and evaluate care plans; adjust as needed.
• Collaborate with interdisciplinary team for comprehensive care.
• Monitor and report changes to medical director and staff.
• Maintain accurate, detailed records of care and treatments.

About the Role:

As a Registered Nurse (RN) at The Avenue at Strongsville, you will have the opportunity to make a meaningful difference in the lives of our residents and their families. You will be responsible for providing direct patient care, assessing patient needs, developing and implementing individualized care plans, and collaborating with our interdisciplinary team. With a patient-centered approach, you will help our residents achieve their maximum potential and maintain their independence.

Responsibilities:

• Provide high-quality patient care in accordance with our facility's standards and regulations
• Conduct initial and ongoing assessments of patient needs, identify risks, and develop effective care plans
• Implement and evaluate the effectiveness of care plans, making adjustments as necessary
• Collaborate with other healthcare professionals to ensure comprehensive care
• Monitor and report changes in patient condition to the medical director and other relevant personnel
• Maintain accurate and detailed records of patient care and treatment
