Job summary

Home Sweet Home Health Agency in Arizona is seeking a qualified Registered Nurse (RN) to join our Home Health team across Maricopa County and surrounding East Valley communities. This role focuses on mobile, in-home care for geriatric and adult patients, delivering skilled nursing services with independence and compassion.

Candidates should hold a current AZ RN license, have at least 1 year of RN experience (home health preferred), and be willing to travel.

Qualifications

• Current Arizona Registered Nurse (RN) license in good standing.
• BSN preferred.
• Minimum of 1 year of RN experience.
• Home Health nursing experience strongly preferred.
• Experience with geriatric patient populations preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and organizational abilities.
• Valid driver’s license, reliable transportation, and ability to travel throughout the East Valley areas.
• Ability to work independently while delivering compassionate mobile care.

Responsibilities

• Conduct comprehensive nursing assessments in patients’ homes.
• Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring.
• Develop, implement, and update individualized home health care plans.
• Educate patients and caregivers on disease management, medications, safety, and ongoing care needs.
• Collaborate with physicians, case managers, therapists, and interdisciplinary care teams.
• Complete accurate and timely clinical documentation in compliance with agency and regulatory standards.
• Monitor patient progress and communicate changes in condition to the care team.
• Support improved patient outcomes, independence, and quality of life.
