Job summary

Home Sweet Home Health is seeking compassionate Registered Nurses to join our growing Home Health team in the West Valley and surrounding Arizona communities. This PRN/home health RN role offers flexible scheduling, autonomy, and meaningful one-on-one patient care in patients' homes.

Ideal candidates have an AZ RN license, at least 1 year of RN experience (home health preferred), and comfort delivering mobile care to geriatric and medically complex patients across Maricopa County.

Qualifications

• Current Arizona RN license in good standing.
• Minimum 1 year of RN experience; home health preferred.
• Experience with geriatric patients and mobility required.

Responsibilities

• Perform skilled nursing assessments and in-home visits across Maricopa County.
• Deliver wound care, medication management, and monitoring.
• Develop and update individualized home health care plans.
• Educate patients and families on disease management and safety.
• Collaborate with physicians, therapists, and case managers.
• Document care accurately and timely in the electronic medical record.
• Monitor progress and communicate changes to the care team.
• Support optimal patient outcomes and independence.

Home Health RN Responsibilities

As a Home Health Registered Nurse (RN), you will provide skilled nursing services throughout Maricopa County and surrounding Arizona communities. Responsibilities include:

• Conduct comprehensive nursing assessments in patients’ homes
• Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring
• Develop, implement, and update individualized home health care plans
• Educate patients and caregivers on disease management, medications, safety, and ongoing care needs
• Collaborate with physicians, case managers, therapists, and interdisciplinary care teams
• Complete accurate and timely clinical documentation in compliance with agency and regulatory standards
• Monitor patient progress and communicate changes in condition to the care team
• Support improved patient outcomes, independence, and quality of life

Qualifications

To be considered for this RN Home Health position in Arizona, candidates should meet the following requirements:

• Current Arizona Registered Nurse (RN) license in good standing
• Graduate of an accredited nursing program (BSN preferred)
• Minimum of 1 year of RN experience required
• Home Health nursing experience strongly preferred
• Experience working with geriatric patient populations preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and organizational abilities
• Valid driver’s license, reliable transportation, and ability to travel throughout the East Valley, Scottsdale, and North Phoenix areas
• Ability to work independently while delivering compassionate mobile care
