Job summary

Home Sweet Home Health is seeking compassionate and skilled Registered Nurses (RNs) to join our growing Home Health team in Maricopa County, AZ. This PRN/home health RN position offers flexible per diem scheduling and a chance to deliver high-quality mobile care to geriatric and adult patients in patients' homes.

The ideal candidate has an Arizona RN license in good standing, at least 1 year of RN experience (home health preferred), and willingness to travel across the East Valley, Scottsdale,

Qualifications

• Current Arizona RN license in good standing.
• Graduate of an accredited nursing program; BSN preferred.
• Minimum of 1 year RN experience; home health experience preferred.
• Ability to travel throughout East Valley, Scottsdale, and North Phoenix areas.

Responsibilities

• Provide skilled nursing services in patients' homes across Maricopa County and surrounding AZ communities.
• Deliver wound care, medication management, treatments, and patient monitoring.
• Develop, implement, and update individualized home health care plans.
• Educate patients and caregivers on disease management, medications, safety, and ongoing care needs.
• Collaborate with physicians, case managers, therapists, and interdisciplinary teams.
• Complete accurate clinical documentation in compliance with agency standards.
• Monitor patient progress and communicate changes in condition to the care team.
• Support improved patient outcomes, independence, and quality of life.

Home Health RN Responsibilities

As a Home Health Registered Nurse (RN), you will provide skilled nursing services throughout Maricopa County and surrounding Arizona communities. Responsibilities include:

• Conduct comprehensive nursing assessments in patients’ homes
• Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring
• Develop, implement, and update individualized home health care plans
• Educate patients and caregivers on disease management, medications, safety, and ongoing care needs
• Collaborate with physicians, case managers, therapists, and interdisciplinary care teams
• Complete accurate and timely clinical documentation in compliance with agency and regulatory standards
• Monitor patient progress and communicate changes in condition to the care team
• Support improved patient outcomes, independence, and quality of life

Qualifications

To be considered for this RN Home Health position in Arizona, candidates should meet the following requirements:

• Current Arizona Registered Nurse (RN) license in good standing
• Graduate of an accredited nursing program (BSN preferred)
• Minimum of 1 year of RN experience required
• Home Health nursing experience strongly preferred
• Experience working with geriatric patient populations preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and organizational abilities
• Valid driver’s license, reliable transportation, and ability to travel throughout the East Valley, Scottsdale, and North Phoenix areas
• Ability to work independently while delivering compassionate mobile care
