Job summary

Angels Care Home Health is seeking an experienced RN Case Manager to coordinate comprehensive home health care for patients in Salina, KS. You will conduct admission visits, determine eligibility for home care services, and develop individualized plans of care in collaboration with the care team.

The role requires an RN license, a valid driver’s license, CPR, and reliable transportation. Prior RN experience and a commitment to professional development are highly valued in a supportive,

Qualifications

• Graduate of an accredited nursing program (Diploma/ADN/BSN).
• Current state RN license.
• Current state driver's license.
• One year RN experience preferred; two years preferred.
• CPR certification required; Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform admission visit to patient at home and assess eligibility for home care services.
• Develop and follow the plan of care with input from other medical disciplines.
• Interact with the care team to ensure quality patient outcomes.
