Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of home care for patients. You will perform admission visits, assess eligibility for home care services, and develop individualized plans of care in collaboration with the patient and multidisciplinary team.

The ideal candidate holds an RN license, a valid driver’s license, and at least 1 year of nursing experience.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year experience as a Registered Nurse, two preferred.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform admission visit to determine eligibility for home care services.
• Develop and follow the plan of care with input from the patient and care team.
