Job summary

Legacy Hospice seeks a Registered Nurse Case Manager (RNCM) to deliver high-quality end-of-life care to patients in their homes in Osceola, AR. The RNCM will coordinate care with an interdisciplinary team, administer medications as ordered, and educate families on symptom management and care expectations.

The role requires an AR-licensed RN with 1 year of hospice/home health experience, strong communication, and ability to work independently and within a team.

Qualifications

• RN licensed in the state of employment (AR).
• BSN or CHPN preferred.
• Minimum 1 year of nursing experience in hospice, home health, or related field.

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans.
• Manage a caseload of hospice patients, coordinating care with the interdisciplinary team.
• Administer medications and treatments per physician orders.
• Educate patients and families on symptom management, medication, and end-of-life care.
• Monitor and update care plans based on ongoing patient assessments.
• Provide emotional and clinical support to patients and caregivers.
• Communicate with physicians, care partners, and community resources.
• Maintain accurate, timely clinical documentation.
• Participate in interdisciplinary team (IDT) meetings and on-call rotation.
• Supervise ancillary staff and ensure continuity of care.

What You’ll Do

• Strong interpersonal, clinical, and organizational skills
• Active CPR/BLS certification
• Valid driver’s license, auto insurance, and reliable transportation
• Ability to work independently and with a team
