Job summary

Legacy Hospice, Inc. in New Albany, MS is seeking a compassionate Registered Nurse Case Manager (RNCM) to deliver high-quality end-of-life care in patients' homes and guide families with empathy and respect.

The RNCM will manage a caseload, coordinate with the interdisciplinary team, administer medications per orders, educate families on symptom management and care planning, and document care accurately.

Qualifications

• Registered Nurse licensed in the state of employment (MS).
• BSN or Certified Hospice/Palliative Nurse (CHPN) preferred.
• Minimum 1 year of nursing experience in hospice, home health, or related field.
• Strong interpersonal, clinical, and organizational skills.
• Active CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans.
• Manage a caseload of hospice patients, coordinating care with the interdisciplinary team.
• Administer medications and treatments per physician orders.
• Educate patients and families on symptom management, medication, and end‑of‑life care.
• Monitor and update care plans based on ongoing patient assessments.
• Provide emotional and clinical support to patients and caregivers.
• Communicate with physicians, care partners, and community resources.
• Maintain accurate, timely clinical documentation.
• Participate in interdisciplinary team (IDT) meetings and on‑call rotation.
• Supervise ancillary staff and ensure continuity of care.
