Job summary

Pathways Healthcare is seeking a full-time registered nurse to join our in-home care team across Quincy, MA and surrounding areas. The role emphasizes direct patient care, care planning, and collaboration with physicians to support aging in the home.

The ideal candidate has MA licensure, 2+ years in geriatrics, a nursing degree, and excellent bedside manners with strong communication. A valid driver's license and reliable vehicle are required.

Qualifications

• Active Nursing license in Massachusetts is required.
• Minimum 2+ years of nursing experience with geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and strong communication skills.
• Working knowledge of healthcare laws and regulations.
• Current BLS CPR certification.
• Valid driver’s license and own a reliable, insured vehicle.

Responsibilities

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

• Salary Range: $72,000 - $90,000

• Possess a valid driver’s license and own a reliable, insured vehicle.
