Job summary

Diamond Care Group is seeking licensed RNs for full-time and PRN home health positions in Florida. You will perform comprehensive assessments, develop care plans, and coordinate with a multidisciplinary team to deliver quality patient care.

The role requires a Florida RN license, at least 1 year of home health experience, and strong communication skills. Level 2 background screening in compliance with Florida AHCA regulations is required prior to employment.

Qualifications

• Current Registered Nurse (RN) license in Florida.
• Minimum of 1 year experience in home health nursing.
• Knowledge of nursing care methods and safety procedures.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage a daily schedule.
• Familiar with EHR systems such as Kinnser.

Responsibilities

• Perform comprehensive patient assessments and develop care plans with the care team.
• Review medications and vital signs as part of evaluation tasks.
• Coordinate care by making referrals when appropriate and documenting rationale.
• Communicate patient status and updates to the clinical manager and team.
• Educate patients and caregivers to promote independence and self-care.
• Coordinate with physicians and therapists involved in the care plan.
• Supervise and support LPNs to maintain high standards of care.
• Perform additional duties to support patients and team operations.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated.
• Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates.
• Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
• Perform additional duties as needed to support patients and team operations.

Qualifications

• Current Registered Nurse (RN) license in the state of Florida
• Must have at least 1 year experience as a home health nurse
• Excellent knowledge of nursing care methods and procedures
• In-depth knowledge of health and safety guidelines and procedures
• A team player with excellent communication and interpersonal skills
• Outstanding organizational and multi-tasking skills
• Patient with excellent problem-solving skills
• Familiar with EHR such as Kinnser.
• Ability to work independently, manage a daily schedule, and travel between patient homes
