Job summary

A community health center in Pittsburgh is seeking a dedicated Registered Nurse (RN) to join their team. Qualifications

  • Minimum 2 years of experience as a Registered Nurse preferred.
  • Licensure as a professional nurse in the Commonwealth of Pennsylvania.
  • Excellent organizational skills and ability to deal effectively with the public.
  • Knowledge of medical terminology, ICD-10, and CPT coding preferred.
  • CPR certified.
  • All CDC recommended vaccinations required.

Responsibilities

  • Perform triage, answer questions, and screen patients.
  • Conduct in-person nurse’s visits, including patient intake.
  • Collect specimens and perform laboratory testing as directed.
  • Administer injections and document activities in EHR.
  • Review lab results and refer abnormal results to Providers.
  • Coordinate chronic disease programs and manage follow-up care.
  • Maintain confidentiality and compliance with HIPAA.

Overview

Squirrel Hill Health Center, a Federally Qualified Health Center (FQHC), is seeking a dedicated and professional Registered Nurse (RN) to join our team. The RN will be responsible for triaging patient phone calls and walk-in patients, conducting nurse’s visits, managing chronic disease programs, providing health education, and performing other RN duties as needed. The ideal candidate will ensure the smooth flow of both people and information through the SHHC system and work collaboratively with all members of the SHHC Team to provide high-quality patient care.

Key Responsibilities
  • Perform triage, answer questions, screen, and determine the need for immediate and future care for patients of all ages.
  • Conduct in-person nurse’s visits, including patient intake, vital signs, and medication history collection.
  • Collect specimens and perform laboratory testing as directed.
  • Administer injections and document activities in the Electronic Health Record (EHR).
  • Review lab results and refer abnormal results to Providers.
  • Act as a liaison between patients, physicians/NPs/PAs, and other entities.
  • Coordinate chronic disease programs and manage follow-up care.
  • Answer incoming calls and gather information for direct notification of clinicians.
  • Coordinate prescription refills, orders, and referrals.
  • Support population health quality initiatives.
  • Work with complementary SHHC teams to support patients and their care plans.
  • Maintain confidentiality and compliance with HIPAA.
  • Refer patients to the Care Navigation Team for necessary social services.
  • Coordinate clinics and offsite prevention services as directed.
Core Competencies
  • Strong RN job skills, knowledge, and abilities.
  • Excellent critical thinking, interpersonal, and communication skills.
  • Proficiency in EHR, Microsoft Word, Teams, Outlook, and Excel.
  • Ability to manage complex patients and situations, often using interpretation services and technology.
  • Fluency in a second language is preferred.
Qualifications
  • Minimum 2 years of experience as a Registered Nurse preferred
  • Licensure as a professional nurse in the Commonwealth of Pennsylvania.
  • BSN preferred.
  • Excellent organizational skills and ability to deal effectively with the public.
  • Ability to work in any SHHC location onsite.
  • Flexibility to accommodate occasional schedule changes for critical coverage.
  • Knowledge of medical terminology, ICD-10, and CPT coding preferred.
  • CPR certified.

  • All CDC recommended vaccinations required.
