Job summary

VIVA USA Inc. in Phoenix, AZ is seeking Registered Nurses for day and night shifts in a full-time agency staffing role. Responsibilities include administering medications, conducting resident assessments, and coordinating with the interdisciplinary team to ensure high-quality geriatric care.

The position requires an AZ RN license and 2+ years of SNF/LTC experience; shifts are 12 hours, with a block schedule of three days one week and four days the next, every other weekend. Onsite only.

Qualifications

• RN license in AZ is required.
• Minimum of 2 years of SNF/LTC experience preferred.
• Ability to work with interdisciplinary team.

Responsibilities

• Administer medications and treatments on assigned unit per physician orders.
• Receive and deliver information about residents' medical, social, functional needs.
• Participate in shift handoffs and reporting.
• Communicate with residents, families and staff to ensure appropriate care.
• Conduct initial admissions nursing assessment and assist with MDS and care plans
• Maintain clinical and administrative documentation of diagnoses, assessments and progress
• Supervise and assist staff for ADLs (bathing, dressing, eating, toileting, mobility)
• Participate in in-service training and development programs

Job description

Looking for Day and Night Shift RN's

• Administer medication and perform treatments on an assigned unit, as ordered by the physician and within scope of license
• Receives and delivers a variety of information regarding the residents’ medical, social, functional and other needs
• Participates in the receiving and giving of shift reports.
• Communicate with residents, family and staff as necessary to ensure the resident receives appropriate care from the interdisciplinary team
• Performs the initial admission nursing assessment on a newly admitted resident; assists with the development of the Minimum Data Set (MDS) assessments, ongoing quarterly reviews and care plans
• Maintains appropriate clinical and administrative documentation regarding diagnosis, assessments, treatment and summary of client progress
• Leads and performs limited supervision of subordinate staff and assists as needed with direct resident care for the completion of Activities of Daily Living (ADLs) including bathing, dressing, eating, toileting, personal hygiene and mobility
• Conducts and participates in in-service training and staff development programs for regularly employed staff and other duties as assigned.
• Utilizes client management system concepts and tools for problem solving and continuous work/process improvements and efficiency Other duties as assigned, as related to the position.

• Developing and carrying out patient care plans within a team nursing concept

Ability to:

• Learn, implement and monitor client management system methods, and to use client management system concepts and tools for problem solving and continuous work/process improvement and efficiency.

Selective Preference(s):

• Open to recent graduates and less than 1 year of experience in skilled nursing facility or long-term care experience.

Requirements:

• RN license in AZ
• Minimum of 2 years of preferred SNF/LTC experience to the position's requirements

• Schedule: 12hr shifts 6:00am-6:30pm / 6:30pm - 6:00am
• Looking for full-time agency staff willing to take a block schedule of three days one week and four days the opposite week, every other weekend.
