Job summary

Transitions Care is seeking a compassionate Registered Nurse to provide hospice visit care in patients’ homes and facilities. You’ll perform skilled nursing assessments, manage symptoms, administer medications, and implement individualized care plans focused on comfort and quality of life.

You will educate and support patients and families, coordinate closely with an interdisciplinary hospice team, and ensure timely documentation.

Qualifications

• Licensed RN with hospice or home health experience.
• Strong clinical judgment and compassionate patient care.
• Excellent communication with patients, families, and team.

Responsibilities

• Provide hospice visit care in patients' homes and facilities.
