Position Overview

We are currently seeking a compassionate and experienced Registered Nurse (RN) to join our Salem-based Home Health team. In this role, you will assess patient needs, develop care plans, provide skilled nursing interventions, and educate patients and their families on managing their health.

Key Responsibilities

• Conduct comprehensive initial and follow-up patient assessments.
• Develop and revise individualized care plans to reflect patient progress.
• Administer treatments and medications as prescribed.
• Provide patient and caregiver education to promote independence and recovery.
• Maintain clear communication with physicians and interdisciplinary care teams.
• Document patient care accurately and in a timely manner.
• Supervise Home Health Aides and evaluate their care delivery.
• Participate in on-call rotations as needed.
  
  

Qualifications

• Current Oregon RN license and BLS certification.
• Graduate of an accredited nursing program.
• Minimum of one year of recent acute care or home health experience preferred.
• Bachelor's degree in Nursing is a plus.
• Must possess a valid driver's license, reliable transportation, and insurance.
  
  

Compensation

• Hourly Rate: $55-$59/hour, negotiable based on years of service and home health experience.
• Mileage Reimbursement provided for travel between patient homes. Employee Benefits
