Summary

This Registered Nurse Remote Patient Monitoring Home Telehealth position is located at the Lebanon VAMC - Lebanon, PA.

Qualifications Basic Requirements:

• English Language Proficiency
• In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless proficient in basic written and spoken English.
• Graduate of an accredited school of professional nursing (ACEN or CCNE accreditation at the time of completion).
• OR individuals in a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's degree in Nursing, with opportunity to become registered prior to completion.
• Upon obtaining a State license, appointment can be temporary and later converted to permanent.
• For foreign graduates, possession of current, unrestricted registration meets graduation requirements; foreign higher education requires degree validation.
• Current unrestricted registration as a professional nurse in the US or District of Columbia.
• Graduate Nurse Technician (GNT) Exception: Candidates without licensure may be appointed temporarily while pursuing licensure.

Duties

• Lead and manage complex patient care in the Remote Patient Monitoring (RPM) Home Telehealth (HT) program.
• Provide care in a cost-effective, least restrictive manner.
• Coordinate care using health informatics, disease management, and HT technologies to improve Veteran health outcomes.
• Assess and review all data from HT patients to establish care plans.
• Triages and assesses vital signs, symptoms, and responses from HT patients.
• Identify and intervene for potential complications to facilitate timely care in various settings.
• Communicate and coordinate with community-based providers and interdisciplinary teams.
• Support self-management through education, coaching, and motivational counseling.
• Evaluate outcomes and document program or service-level results.

• Work Schedule: Monday - Friday, 7:30 a.m. - 4:00 p.m.
