Job summary

Resources for Human Development (RHD) is hiring a Registered Nurse (RN) Staff Nurse to join the Delaware Intensive Case Management team. The Delaware Intensive Case Management program of Resources for Human Development (RHD) provides community-based treatment and support to individuals living with severe and persistent mental illness and co-occurring conditions.

Our team takes an individualized, recovery-focused approach to care, helping participants address behavioral and physical health needs while developing the skills and supports needed to achieve greater stability and independent functioning in the community.

What You’ll Do

As the RN Staff Nurse, you will provide comprehensive nursing care and clinical support to individuals living with severe and persistent mental illness and co-occurring conditions.

You’ll manage medications and treatments, conduct clinical and psychiatric assessments, address physical health needs, educate participants and families, and coordinate care with medical providers and the interdisciplinary treatment team.

Your Responsibilities May Include:

Nursing Assessment & Clinical Care

  • Conduct psychiatric, clinical, and physical health assessments.
  • Provide nursing care and monitor participants' ongoing health status.
  • Conduct vital signs, metabolic screenings, injections, and other ordered treatments.
  • Identify changes in physical or behavioral health and communicate concerns to appropriate providers and team members.
  • Address physical health needs as part of each participant's overall treatment and recovery plan.
  • Provide individualized nursing interventions based on participant needs.

Medication Management

  • Administer and manage medications and prescribed treatments.
  • Educate participants about medications, symptoms, potential health concerns, and disease prevention.
  • Support participants in developing medication management skills.
  • Maintain medication inventory and ensure medications are appropriately stored and documented.
  • Ensure medication practices comply with applicable laws, regulations, and organizational procedures.
  • Communicate changes in medications, treatment, or health conditions to appropriate team members and providers.

Community-Based Support & Education

  • Provide nursing services in participants' homes and other community settings.
  • Assist with activities of daily living training, medication education, and home management when appropriate.
  • Provide supportive counseling to individuals receiving services.
  • Help participants strengthen skills that promote greater health and independent community living.
  • Provide education and consultation to families and natural supports.
  • Advocate for participants and their healthcare needs throughout the community.

Care Coordination

  • Coordinate care with physicians, pharmacies, medical providers, ancillary services, and other community resources.
  • Ensure appropriate follow-through with medical appointments and prescribed care.
  • Refer and connect participants with external medical and community services.
  • Maintain collaborative relationships with external healthcare providers.
  • Participate in treatment planning and interdisciplinary case conferences.
  • Coordinate with the treatment team to help address housing, employment, health, rehabilitation, and community resource needs.

Crisis & Emergency Support

  • Coordinate emergency and crisis intervention services when needed.
  • Maintain composure and exercise sound clinical judgment during crisis situations.
  • Communicate significant changes in participant conditions to appropriate staff and healthcare providers.
  • Participate in required on-call responsibilities.
  • Assist the treatment team in responding to urgent health and behavioral health needs.
  • Work closely with behavioral health professionals and other members of the interdisciplinary team.
  • Train and supervise unlicensed staff on appropriate care procedures and standards.
  • Provide nursing consultation to team members.
  • Communicate clinical updates to ensure coordinated and consistent care.
  • Participate in team meetings, trainings, and care coordination activities.

Documentation, Safety & Compliance

  • Complete clinical documentation accurately and within required timeframes.
  • Maintain confidentiality and follow HIPAA requirements.
  • Ensure compliance with applicable licensing regulations and medication laws.
  • Support infectious disease prevention and control activities.
  • Follow OSHA, fire, safety, security, and organizational requirements.
  • Attend required meetings and trainings.
  • Maintain a professional demeanor and exercise sound clinical judgment.

Qualifications
Required
  • Current Registered Nurse (RN) license in Delaware
  • Graduate of an accredited School of Nursing
  • Pre-employment Tuberculin Skin Test
  • CPR Certification
  • Successful completion of required background investigation
  • Ability to obtain and maintain required security clearance
  • Current Driver’s License
  • Current vehicle registration and access to a functional vehicle
  • Ability to travel throughout the community to provide services
Preferred
  • Bachelor of Science in Nursing (BSN)
  • At least one year of clinical nursing experience
  • Experience in behavioral health, psychiatric nursing, community health, or a related setting
Schedule & Compensation
