Job description

The Child Center of NY is starting a Certified Community Behavioral Health Clinic (CCBHC) through OMH. The CCBHC is to ensure access to coordinated comprehensive behavioral health care. All CCBHCs are required to serve anyone who requests care for mental health or substance use, regardless of their ability to pay, place of residence, or age. This includes developmentally appropriate care for children and youth.

The Child Center of NY is seeking a compassionate and skilled Registered Nurse (RN) to join our team at our Certified Community Behavioral Health Clinic (CCBHC). The nurse will play a key role in ensuring and providing high-quality, patient-centered care to individuals experiencing mental health and substance use disorders. This position involves supervising a team of LPNs and collaborating with an interdisciplinary team to promote wellness, administer treatments, and provide education on behavioral health conditions.

Essential Functions:

  1. Assess clients' needs for physical exams and conduct health assessments, including communicable diseases, tobacco use, vital signs, medication reconciliation, and screenings for medical and behavioral health conditions.
  2. Coordinate medical care, including providing referrals to community medical providers, scheduling appointments, and liaising with external providers to obtain patients' records.
  3. Conduct metabolic monitoring (e.g., blood pressure, weight, and BMI).
  4. Obtain and ensure processing of lab work for patients drawn both internally and externally.
  5. Administer medications, including injectable psychotropic medications, following prescribed guidelines and safety protocols.
  6. Provide basic first aid and manage medical emergencies and crisis situations appropriately.
  7. Monitor and document patient responses to treatments and medications, reporting concerns to the prescribing provider and treatment team.
  8. Provide patient education and counseling, both individually and in groups, regarding medications, chronic conditions, behavioral health diagnoses, healthy eating, exercise, family planning, safe sex practices, harm reduction strategies for substance use, and general wellness.
  9. Engage in outreach and follow-up to promote patient engagement and adherence to treatment plans.
  10. Support crisis intervention efforts, including de-escalation and collaboration with emergency services when necessary.
  11. Coordinate with pharmacies to ensure timely access to prescribed treatments.
  12. Perform and submit prior authorizations for medications, procedures, and other treatment services, including reviewing denials and obtaining additional information that could alter the initial decision.
  13. Collaborate with psychiatric providers, therapists, case managers, and other healthcare providers to coordinate comprehensive care.
  14. Maintain accurate and timely documentation in the electronic health record (EHR).
  15. Participate in care team meetings, case consultations, and treatment planning discussions.
  16. Ensure compliance with state and federal regulations, as well as CCBHC quality and reporting standards.
  17. Fulfilling other responsibilities that may emerge.

Minimum Qualifications:

  1. Graduate of an accredited nursing program (Associate's or Bachelor's degree in Nursing).
  2. Active RN license in the state of New York.
  3. Training and experience working with people of all ages in behavioral health, mental health, or substance use treatment settings.
  4. CPR/BLS certification required.
  5. Strong communication, problem-solving, and teamwork abilities.
  6. Ability to work amongst diverse social, cultural, and economic groups.
  7. Excellent time management, phone, and face-to-face engagement skills.

Qualifications Preferred:

  1. Certification in Psychiatric Nursing.
  2. Familiarity with trauma-informed care, motivational interviewing, and crisis intervention techniques preferred.
  3. Language skills in Spanish, Mandarin, Cantonese, Bengali, Hindi, Korean, etc., may be required depending on client needs.

Supervisory Responsibilities:

  1. Supervise a team of LPNs providing comprehensive care and primary care coordination for CCBHC clients.
  2. Develop, implement, and monitor health assessment, monitoring, and coordination processes and standards of excellence in collaboration with the Medical Director.
  3. Work as part of an interdisciplinary management team to drive integration of physical and emotional well-being.
  4. Establish and serve as the lead connection to contracted pharmacies and labs for daily needs.

Physical demands include walking, sitting for long periods, using hands for handling objects, reaching, balancing, stooping, talking, hearing, and occasionally lifting/moving up to 20 pounds.
