Job summary

ComForCare Home Care in NYC is seeking a thoughtful, organized registered nurse to join our team on a part-time basis (15–25 hours per week). You will perform in-home assessments, coordinate care with HHAs/PCAs, and ensure high-quality client care while traveling to private client homes throughout NYC.

This role combines clinical insight with administrative collaboration, requiring NYS RN licensure, CPR/BLS, and strong organizational skills.

Qualifications

• Unencumbered New York State RN license.
• Current CPR/BLS certification.
• Ability to travel to private client homes throughout NYC.
• Strong organizational skills and attention to detail.
• Ability to work independently while following procedures.

Responsibilities

• Perform personalized in-home initial assessments and reassessments, developing and updating care plans.
• Organize and schedule assessments, reassessments, supervisory visits, and competency evaluations.
• Communicate with clients and family members and answer clinical questions.
• Track changes in client condition and follow up with the team as needed.
• Provide orientations, ongoing education, and in-field support to HHAs and PCAs.

Part time RN: 15-25 hours per week

$45/hour starting rate, with an increase after 3 months

Location:

Primarily office-based, with travel to private client homes throughout NYC as needed

ComForCare Home Care is opening up the opportunity for a thoughtful, organized RN to join our Team. This role is a great fit for a nurse who takes the time to seek an in-depth understanding of both the clinical and administrative aspects of our operations, representing our commitment to delivering the highest standard of care.

You’ll work closely with private clients and their families while leading our hardworking HHAs/PCAs, conducting and scheduling assessments, supervisory visits, and caregiver competencies. While your role can be carried out quite independently, you’ll stay connected to the entire team while we all work together to make a difference in our NYC community!

Responsibilities:

• Organize and schedule assessments, reassessments, supervisory visits, aide orientations, and competency evaluations.
• Communicate with clients and family members and answer clinical questions.
• Track changes in client condition, communicate concerns to the appropriate team members and caregivers, and follow up as needed.
• Provide orientations, ongoing education, competency evaluations, and in-field support to HHAs and PCAs.

Requirements

• Unencumbered New York State RN license
• Current CPR/BLS certification
• Ability to travel for assessments and visits in Brooklyn and Manhattan, with occasional travel to Queens and the Bronx
• Strong organizational skills
• A detail-oriented approach to clinical assessments and documentation
• Someone who can follow established procedures while using sound judgment and working independently
