Job summary

Affirmed Home Care, Inc. in New York City is seeking a Registered Nurse (RN) to provide compassionate in-home care across all five boroughs. Earn $55–$65 per hour, commensurate with experience in a home care setting.

You will perform client assessments, administer medications, monitor health, and educate families. CPR/BLS certification and NY RN license are required, with strong clinical skills and independence expected.

Qualifications

• Active RN license in New York.
• 1–2 years of nursing experience, preferably home health or geriatrics.
• Strong clinical skills and independent work ability.
• Excellent communication to build rapport with clients/families.
• CPR/BLS certification is required.

Responsibilities

• Conduct comprehensive client assessments and create care plans.
• Administer medications, treatments, and nursing procedures.
• Monitor health status and adjust plans as needed.
• Educate clients and families on disease management and safety.
• Perform advanced nursing procedures including wound care.
• Document care in accordance with regulatory guidelines.
• Ensure compliance with state and federal regulations.
• Provide emotional support to clients and families.
• Stay updated with nursing best practices.

Key Responsibilities:

Conduct comprehensive client assessments to create personalized care plans.

Administer medications, treatments, and nursing procedures as required.

Monitor clients’ health status (vital signs, progress) and adjust care plans as needed.

Educate clients & families on disease management, medication use, and home safety.

Perform advanced nursing procedures including wound care, IV therapy, and catheter care.

Document all care provided in compliance with regulatory guidelines.

Ensure compliance with state and federal healthcare regulations.

Provide emotional support and guidance to clients and their families.

Stay up-to-date with nursing best practices and participate in professional development.

What We’re Looking For:

Active RN License in the state of New York.

1-2 years of nursing experience (home health, geriatrics, or similar preferred).

Strong clinical skills and ability to perform nursing procedures independently.

Excellent communication skills and the ability to build rapport with clients and families.

Self-motivated and organized, with the ability to work independently in a home care setting.

Reliable transportation and a valid driver’s license.

CPR and BLS certification (required).

Experience with electronic health records (EHR) systems (preferred).
