Job summary

Green Key Resources is seeking a bilingual Spanish Field Registered Nurse for a Brooklyn-based home care organization. This field-based role focuses on assessing clients, coordinating care, developing and monitoring plans of care, and supporting patients and their caregivers in the home.

Responsibilities include in-home assessments, care coordination with families and providers, patient education, supervising Home Health Aides, and maintaining clinical documentation.

Qualifications

• Current New York State RN license.
• Home care or CHHA experience strongly preferred.
• 2+ years of home health experience preferred.
• 3+ years of relevant nursing experience.
• BLS certification.
• Strong assessment, organization, communication, and care coordination skills.
• Comfortable traveling to clients' homes throughout NYC.

Service Area:

Brooklyn (primarily Bushwick)

• Monday–Friday, 35 hours/week
• Flexibility to work weekends when needed
• Approximately 5 visits per day when in the field
