Job summary

Amazing Home Care, LLC in New York City is seeking an experienced Registered Nurse (RN) to perform patient visits across all five boroughs. NEW GRADUATES CAN APPLY!

The role requires coordinating and supervising HHAs, initiating visits, and reassessing patient needs under physician orders. Responsibilities include observing and documenting progress, notifying physicians of changes in condition, and educating patients and caregivers on disease processes, medications, and home health care plans.

Qualifications

• Must be able to travel to multiple patients per day.

Responsibilities

Amazing Home Care is actively seeking a motivated and experienced Registered Nurse (RN) to perform patient visits throughout all five boroughs for our Licensed Home Care Agency. Preferably, the candidate should be bilingual in English and Spanish/Creole/Cantonese/Mandarin; however, it is not arequirement.

Job Summary:

• Demonstrate proficient skills to admit and reassess home health patients in regards to physician orders

• Coordinate and supervise HHAs in the delivery and discharge of patient care

• Initiate home care visits and reassess patient needs and progress on a regular basis

• Initiate the plan of care under doctor’s orders

• Observe, assess and document all patient progress

• Notify physician and other personnel of changes in conditions of patients

• Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plan as well as in-home health care

Qualifications/Educational Requirements:

• Current NYS Registered Nurse License

• Must be vaccinated against COVID-19

• Work positively and favorably with patients, families, and staff

• Excellent communication and time-management skills

• Ability to travel to multiple patients per day

• Bilingual English & Spanish
