Job summary

Legacy Hospice is seeking a dedicated Registered Nurse Case Manager to provide compassionate end‑of‑life care to patients in home settings. You’ll collaborate with an interdisciplinary team to develop care plans, coordinate services, and educate families on symptom management.

The role requires an active RN license in Missouri, 1+ year hospice/home health experience, and strong communication skills. A BSN or CHPN is preferred and pre-employment drug screening is required.

Qualifications

• RN licensed in the state of employment.
• BSN or CHPN preferred.
• Minimum 1 year of nursing experience in hospice, home health, or related field.
• Strong interpersonal, clinical, and organizational skills.
• Active CPR/BLS certification.
• Valid driver’s license, auto insurance, and reliable transportation.
• Ability to work independently and with a team.
• Pre-employment drug screening is required.

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans.
• Manage a caseload of hospice patients, coordinating care with the interdisciplinary team.
• Administer medications and treatments per physician orders.
• Educate patients and families on symptom management, medication, and end-of-life care.
• Monitor and update care plans based on ongoing patient assessments.
• Provide emotional and clinical support to patients and caregivers.
• Communicate with physicians, care partners, and community resources.
• Maintain accurate, timely clinical documentation.
• Participate in IDT meetings and on-call rotation.
• Supervise ancillary staff and ensure continuity of care.
