Job summary

HMG Healthcare in Mission, TX, is seeking a Charge Nurse to deliver compassionate nursing care to residents in our post-acute setting. You will administer medications, monitor vital signs, and support daily living activities under supervision of RNs and physicians.

Collaborate with interdisciplinary teams to develop care plans, maintain accurate records, and advocate for resident comfort and dignity. A strong communicator with a TX LVN/RN license and BLS is essential.

Qualifications

  • Current and valid LVN or RN license in Texas.
  • Completion of an accredited vocational nursing program.
  • BLS certification.
  • Experience in long-term care or rehabilitation settings is preferred.

Responsibilities

  • Provide direct nursing care, including medications and treatments as prescribed.
  • Monitor vital signs, document changes, and report significant findings.
  • Assist residents with activities of daily living and mobility.
  • Collaborate with RNs, physicians, and other professionals to update care plans.
  • Maintain accurate patient records in line with policies and regulations.
  • Educate residents and families on health maintenance and prevention.
  • Ensure a safe, clean environment following infection control protocols.

About the Role:

As a Charge Nurse at Mission Nursing and Rehabilitation , you will play a critical role in delivering compassionate and high-quality nursing care to our residents. Your primary focus will be to support the health and well‑being of patients by administering medications, monitoring vital signs, and assisting with daily living activities under the supervision of registered nurses and physicians. You will collaborate closely with interdisciplinary teams to develop and implement individualized care plans that promote recovery and comfort. This role requires a strong commitment to patient advocacy, ensuring that each resident’s needs and preferences are respected and met with dignity. Ultimately, your contributions will directly impact the quality of life and health outcomes of the individuals entrusted to our care.

Minimum Qualifications:
  • Current and valid Licensed Vocational Nurse (LVN) or RN license in the state of Texas.
  • Completion of an accredited vocational nursing program.
  • Basic Life Support (BLS) certification.
  • Demonstrated ability to perform nursing duties in a long‑term care or rehabilitation setting.
  • Strong communication and interpersonal skills.
Preferred Qualifications:
  • Experience working in a nursing home or rehabilitation facility.
  • Additional certifications such as IV therapy or wound care.
  • Familiarity with electronic health record (EHR) systems.
  • Training in geriatric nursing or chronic disease management.
  • Ability to speak multiple languages relevant to the resident population.
Responsibilities:
  • Provide direct nursing care to residents, including administering medications and treatments as prescribed.
  • Monitor and record vital signs, symptoms, and changes in patient conditions, reporting significant findings to the healthcare team.
  • Assist residents with activities of daily living such as bathing, dressing, and mobility support.
  • Collaborate with registered nurses, physicians, and other healthcare professionals to develop and update individualized care plans.
  • Maintain accurate and detailed patient records in compliance with facility policies and regulatory requirements.
  • Educate residents and their families on health maintenance and disease prevention strategies.
  • Ensure a safe, clean, and comfortable environment for residents by adhering to infection control and safety protocols.
