Job summary

Tennessee Quality Care is seeking a compassionate Registered Nurse (RN) for full-time Home Health in Martin, TN. Join a team dedicated to delivering high-quality in-home patient care, improving independence and comfort at home.

Responsibilities include comprehensive nursing assessments, care planning, and coordination with the interdisciplinary team. CPR certification, TN RN license, and reliable transportation are required, with benefits and supportive work-life balance offered.

Qualifications

• Graduate from an accredited registered nursing program and licensed in the state
• At least one year clinical practical nursing experience preferably in ICU, rehab or medical-surgical nursing (preferred)
• Home health experience preferred
• Possess and maintain valid CPR certification
• Must have reliable transportation, current driver’s license and appropriate automobile insurance

Responsibilities

• Provide visits as assigned by the Clinical Services Manager, including assessment and planning, admissions processing, implementation and evaluation of the nursing process
• Obtain data and assess information from patient and family factors affecting needs and coordinate intervention with the Interdisciplinary Team
• Initiate communication with attending physician and other agencies to coordinate optimal care
• Provide visits to assess patient needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by physician
• Perform clinical pronouncement of death and provide family support as applicable to licensure
• Maintain accurate patient records, charts, progress notes, reports and care plans timely

RN What You’ll Do:

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patient's needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications:

• Registered Nurse
  • Graduate from an accredited registered nursing program and licensed in the state
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
  • Home health experience preferred
  • Possess and maintain valid CPR certification.
  • Must have reliable transportation, current driver's license and appropriate automobile insurance
