Key Responsibilities

• Participate actively in the Interdisciplinary Team (IDT), contributing assessment insights, care planning recommendations, and ongoing care coordination.
• Conduct comprehensive nursing assessments (physical, psychosocial, and behavioral) in various settings including clinics, homes, day centers, virtual visits, and long-term care facilities.
• Collaborate with medical providers to deliver personalized care to a panel of PACE participants in line with care plans.
• Administer medications and therapeutic interventions accurately, responding promptly to physician orders and participant needs.
• Manage triage independently in outpatient settings and escalate care appropriately with medical providers.
• Support case management during care transitions, maintaining communication with hospital and long-term care case managers.
• Educate participants, caregivers, and team members to support personalized care plan implementation and success.
• Participate in rotating on-call schedules, providing after-hours triage support via phone.

Schedule & Location

• Full-time, on-site position
• Monday through Friday, daytime hours
• Based in Downtown Los Angeles
