The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and agency policies.

• Coordinate care with healthcare professionals and help maintain continuity of care by planning and sharing information with physicians, staff, patients, families, and community resources within a Point of Care setting.
• Conduct initial and comprehensive nursing evaluations, determine primary care focus, develop care plans within state guidelines, and ensure accurate documentation in collaboration with physicians.
• Perform patient assessments at designated times and develop appropriate care plans.
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer source.

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
