The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes as prescribed by physicians, complying with laws, regulations, and agency policies.

Coordinates the care plan with healthcare professionals and ensures continuity of care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources, all within a Point of Care setting.

Performs initial and/or comprehensive nursing evaluations, determines primary care focus, develops the plan of care within state guidelines, and submits accurate documentation in collaboration with physicians.

Conducts patient assessments and develops appropriate care plans.

Ensures patients meet home health eligibility and medical necessity guidelines.

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to transportation
