Job summary

Jaffrey Rehabilitation and Nursing Center is seeking an RN/LPN to provide direct nursing care to residents in a warm, family-friendly setting.

Active RN or LPN license required; one year of experience preferred. You will administer medications, assist physicians with procedures, monitor vital signs, and support admissions, transfers and discharges in the Jaffrey, New Hampshire community, with a commitment to compassionate patient care.

Qualifications

• Current RN or LPN licensure is required.
• One year of RN/LPN experience is preferred.
• Ability to work in a resident-focused care setting.
• Excellent communication and teamwork skills.

Responsibilities

• Provide direct nursing care to residents as assigned.

Duties and Responsibilities:

• Provides nursing care to patients as assigned.
• Administers and records medications and treatments as assigned.
• Prepares for and assists Physicians and others with diagnostic procedures and treatments.
• Reinforce supports and provides health care instructions to patients and their families.
• Participates with other team members in planning, implementing and evaluating nursing care and other patient activities.
• Assists in the admission, transfer, and discharge of patients.
• Observes and reports patients’ vital signs, symptoms and general condition to the appropriate individual(s).
• Records on patient charts observation of patient’s responses to nursing care rendered.
• Maintains environmental conditions conducive to the comfort and safety of patients.
• Assists with unit management and/or assume charge of unit as assigned.

Experience:

• RN/LPN Registered Nurse Licensed Practical Nurse:1 year (Preferred)

License:

• RN/LPN Registered Nurse Licensed Practical Nurse (Required)
