Job summary

comForCare in Maryland is seeking a highly skilled registered nurse to join our Assistive Care team and provide in-home patient care. You will coordinate and oversee all aspects of home care, educate families, and collaborate with care teams to implement care plans.

A compassionate approach and strong clinical skills are essential. The role requires an active RN license, 12 months of RN experience, and a valid driver's license.

Qualifications

• Active RN license in the state of practice (Maryland).
• Minimum 12 months of RN experience.
• Valid driver’s license and auto insurance in state of residence.
• Basic computer skills including MS Word and Outlook.
• Current CPR certification.

Responsibilities

• Coordinates and oversees all aspects of patients’ home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Other duties as assigned.

Qualifications

Compensation: $55.00 per hour
