Job summary

ComForCare is seeking a Registered Nurse to travel to clients' homes for scheduled, non-clinical nursing assessments and to collaborate with the office team to support care delivery. There is no direct caregiving or bedside responsibilities, and you will conduct verbal and observational assessments with clients and families, review medical history and daily routines, and maintain thorough documentation.

What you will be doing

• Travel to clients’ homes to complete scheduled, non-clinical nursing assessments
• Conduct verbal and observational assessments with clients and family members

• Develop and update individualized care plans for in-home caregivers to follow
• Serve as a clinical resource to the care team regarding the client’s plan of care
• Maintain accurate and timely documentation of assessments and care plans
