Job summary

ComForCare Home Health Care is seeking a skilled Registered Nurse (RN) to join our assistive care team in home health settings. You will provide compassionate, high-quality care to patients in their homes and coordinate with families and care teams.

The role emphasizes meaningful patient interactions, accurate documentation, and ongoing communication with clinicians. We offer flexibility, supportive teammates, and opportunities to grow within a large national home health care network.

Qualifications

• Current Registered Nurse (RN) license in the state of practice.
• Minimum of 12 months of RN experience.
• Current driver's license, reliable transportation, and vehicle insurance in state of residence.
• Basic computer skills and proficiency in MS Word and Outlook.
• Current CPR certification.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Other duties as assigned.
