Alliance Homecare VNA in Fitchburg, MA is seeking a compassionate Registered Nurse (RN) for home health. The RN will travel to patient homes within our approved service area and provide skilled nursing care aligned with physician orders and clinical guidelines.

We offer full-time or part-time opportunities with a competitive pay rate of $40–$55 per hour, health insurance, and opportunities for professional development. Travel within the service area is required to support patient care.

Qualifications

• Current, active Massachusetts RN license.
• Home health experience is preferred.
• Strong assessment, clinical judgment, and documentation skills.
• Ability to travel to patient homes within the approved service area.
• Reliable transportation to travel to assigned patient homes.

Responsabilités

• Conduct patient assessments and evaluate health conditions.
• Develop, implement, and update individualized nursing care plans.
• Provide skilled nursing care in accordance with physician orders and applicable clinical guidelines.
• Monitor patient progress and report changes to the healthcare team.
• Administer medications and treatments as authorized.
• Educate patients and caregivers about health conditions, medications, and care plans.
• Complete accurate and timely clinical documentation.
• Coordinate care with physicians, therapists, and other team members.
• Follow infection-control procedures and patient privacy requirements.

Responsibilities

• Conduct patient assessments and evaluate health conditions.
• Develop, implement, and update individualized nursing care plans.
• Provide skilled nursing care in accordance with physician orders and applicable clinical guidelines.
• Monitor patient progress and report changes in condition to the appropriate healthcare professionals.
• Administer medications and treatments as authorized and appropriate.
• Educate patients and caregivers about health conditions, medications, and care plans.
• Complete accurate and timely clinical documentation.
• Coordinate care with physicians, therapists, and other members of the healthcare team.
• Follow infection-control procedures, patient privacy requirements, and applicable state and federal regulations.

Qualifications

• Current, active Massachusetts Registered Nurse (RN) license.
• Relevant nursing experience preferred; home health experience is an advantage.
• Strong assessment, clinical judgment, communication, and documentation skills.
• Ability to work independently while coordinating with the healthcare team.
• Reliable transportation and willingness to travel to assigned patient homes within the designated service area.
• Professional, compassionate, and patient-focused approach to care.

Service Area

This position serves assigned patients within our approved service area, which may include Fitchburg, Gardner, Leominster, Lunenburg, Westminster, Hubbardston, Princeton, Sterling, Holden, Worcester, Lancaster, Clinton, Bolton, Berlin, Boylston, West Boylston, Northborough, Shrewsbury, and other nearby communities

Specific assignments and travel expectations will be discussed during the hiring process.
