Eminence Home Care is seeking a compassionate and dedicated Registered Nurse (RN) to join our growing home care team. Qualifications

• Active and unrestricted RN license in the applicable state.
• Minimum of 1 year of nursing experience preferred.
• Prior home care or home health experience preferred.
• Strong assessment and communication skills.
• Ability to work independently and manage multiple patient cases.
• Valid driver's license and reliable transportation.
• Proficiency in EMR and clinical documentation.

Responsabilités

• Conduct initial and ongoing patient assessments in the home.
• Develop, implement, and update individualized care plans.
• Perform supervisory visits to evaluate caregiver performance and patient satisfaction.
• Monitor patients' health conditions and identify changes in care needs.
• Provide education to patients, families, and caregivers.
• Complete accurate and timely clinical documentation.
• Ensure compliance with state regulations and agency policies.
• Collaborate with caregivers, families, physicians, and staff to coordinate care.
• Assist with caregiver training and clinical support as needed.
• Address patient concerns and recommend quality improvements.

Job Summary

Eminence Home Care is seeking a compassionate and dedicated Registered Nurse (RN) to join our growing home care team. The RN will be responsible for conducting patient assessments, developing individualized care plans, supervising caregivers, and ensuring high-quality care is delivered in accordance with state regulations and company standards.

Key Responsibilities
