The Terrace of Delray Beach Nursing and Rehabilitation Center in Florida is seeking a Registered Nurse (RN) to provide high-quality care in skilled nursing and long-term care settings. You will perform assessments, administer medications, and collaborate with the interdisciplinary team to promote healing, safety, and patient well-being.

Applicants must hold an active RN or LPN license, BLS/ACLS, and be committed to compassionate, patient-centered care.

Formación

• Active RN or valid LPN licensure with current state licensure.
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).
• Experience in long-term care or skilled nursing facilities preferred.

Responsabilidades

• Perform patient assessments, including vital signs and health history documentation in EHR.
• Administer medications, injections, and treatments per protocols for safety and efficacy.
• Monitor patients closely with airway management, ventilator care, tube feeding, and pain control.
• Collaborate with interdisciplinary teams for discharge planning and case management.
• Maintain accurate records compliant with HIPAA and long-term care standards.
• Educate patients on medications, wound care, dementia care, and health coaching.

Responsibilities

• Perform patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems.
• Administer medications, injections, and treatments following strict protocols to ensure safety and efficacy.
• Monitor patient conditions closely, utilizing skills in airway management, ventilator care, tube feeding, and acute pain management to respond swiftly to changes.
• Coordinate with interdisciplinary teams for discharge planning, case management, and diagnostic evaluation to optimize patient recovery and long-term care plans.
• Maintain accurate medical records and documentation in compliance with HIPAA regulations and long-term care standards.
• Provide patient education on medication administration, wound care, dementia or Alzheimer's care, and health coaching tailored to individual needs.
• Support infection control practices and aseptic techniques during procedures such as venipuncture, suturing, and specimen collection.

Qualifications

• Must have a valid RN or LPN license with current state licensure; additional certifications such as Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).
• Knowledge of long-term care regulations and experience in skilled nursing facilities, preferred.
• Excellent communication skills for caregiver communication and patient/client interaction; ability to work effectively within multidisciplinary teams.
