Job summary

Elara Caring seeks a compassionate Registered Nurse to provide in-home health services in the Bryan/College Station area. You will perform nursing assessments, develop and update care plans, administer physician-directed care, and collaborate with families and the care team to ensure safety and quality at home.

You will also educate patients and caregivers, document visits in HCHB, and support coordinated, compassionate care across visits. Prior home health and HCHB familiarity are preferred.

Qualifications

• Current Registered Nurse license in the state of practice.
• Graduation from an accredited nursing program.
• At least one year of nursing experience in a clinical setting.
• Home health experience and familiarity with Homecare Homebase (HCHB).
• Valid driver’s license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and moving equipment.

Responsibilities
