Job summary

Boost Home Healthcare-Bradenton is seeking a Registered Nurse (RN) to provide skilled nursing care in patients' homes in Bradenton, FL. You will follow physician orders and individualized care plans under the guidance of our Client Services Director.

Qualifications

• Graduate of an accredited school of professional nursing.
• Currently licensed as a registered nurse in the state of the job posting.
• Minimum of two (2) year of nursing experience in a home health setting.
• Minimum of one (1) year of supervisory experience as a registered nurse.
• Must have current BLS Certification.
• Access to reliable transportation.
• Experience in Oasis.

Responsibilities

• Make the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide those services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.

What You’ll Be Doing
