Job summary

VNA Health Center seeks a registered nurse to deliver comprehensive nursing care across clinic sites and home-based services, including triage, education, immunizations and family planning support.

You will administer medications and vaccines per protocols, document care in the EHR, and coordinate referrals with other health professionals to ensure continuity of care.

Qualifications

• Must be a registered nurse licensed in Illinois.
• Current CPR certification required.
• Experience in community health is preferred.
• Education/experience in child health, immunizations, and/or women’s health is beneficial.

Responsibilities

• Provide nursing care in clinic, community, phone triage, home-based services (home health, hospice, house calls).
• Administer medications and vaccines according to protocols.
• Educate patients and families about plans of care and health maintenance.
• Coordinate referrals and follow-up with other health care providers.

Job description

Provides, coordinates and evaluates effectiveness and outcomes of care to clients presenting for medical/ nursing based services at the VNA Health Center clinic sites (on and off clinic sites to include community and home based services: Home Health, Hospice and House Calls). These clinics and services may include:

• Primary Health Care – comprehensive primary medical care for clients of all ages across the health care continuum. Includes performing procedures related to care (e.g. vitals, treatments as ordered by the MD/NP…), assessment of patients’ needs for counseling, education and referrals, pregnancy options counseling, and family planning care and education.
• Women’s Wellness Clinic – care to women in need of health maintenance and promotion services, family planning, STD counseling, as well as care for acute and chronic illnesses. This may include: assessment and counseling in all aspects of women’s health; immunization assessment, administration and counseling; and performing procedures related to care (e.g. anthropometric measurements, vital signs, and Waived Testing
• Child Health Clinic – care to children in need of routine preventive, and for acute and chronic illnesses. This may include: assessment and counseling in all aspects of child health (physical, developmental, emotional, social, dental and nutritional); immunization assessment, administration and counseling; and performing procedures related to care (e.g. anthropometric measurements, vital signs, and Waived Testing.
• Immunization Clinic – assessment, administration and counseling for the ACIP recommended vaccines for clients from infancy through adulthood.

Reports To: Health Center Coordinator and or Associate Director of Health Center Operations

Primary Functions

• Workassignment (clinic, community, phone/triage, homebased services) isdetermined based upon business need as determined by the AssociateDirector of Health Center Operations or Health Center Coordinator.
• Preparesnursing, medical and lab areas with medical supplies; emergency kits;vaccine; and educational and referral materials as appropriate forspecific clinic services.
• Reviewsthoroughness of Client Registration/ Consent and other consents specificto services to be provided.

Obtains completeclient and family health histories if appropriate to specific clinic services.

Assesses clientneeds, develops and coordinates an individualized plan of care, includinginvolving the client/family in the development of the client’s goals andcollaboration with other health care professionals as necessary.

Initiates nursinginterventions identified in the plan of care, per applicable Standing Ordersand Protocols, evaluating effectiveness and outcomes of care and revising asneeded.

Administersmedications as ordered by the clinic physician or nurse practitioner accordingto applicable protocols, policies and procedures. This includes medicationsrelated to family planning and emergency contraception.

Educates/counselsclient (or parent/guardian, if client is a child) specific to identified needs,plan of care and clinic requirements as described in the appropriate clinicpolicies and procedures. (e.g. Family Planning Program, IBCCP Program,Immunization Program)

Initiatesreferrals to other health care services as needed and completes follow up onreferrals per applicable protocols, policies and procedures.

Call Center(Phone Triage)

Communicateswith patients over the phone regarding symptoms and health concerns anddetermines the type of care needed.

Returnspatient related calls. Communicates and coordinates accordingly with clinicaland provider staff.

Ensurespatient calls are addressed in a timely manner following agency procedure andprotocols.

Provides HomeBased Services (Home Health, Hospice and House Calls) nursing care for all VNApatients, as assigned.

Completes clientclinical record documentation specific to clinic services provided, accordingto applicable policies and procedures, and in a timely manner.

Maintains safehandling, administration and storage of vaccines and medications.

Documentsadministration of medications and vaccines on the appropriate inventory log andEMR.

Understands andapplies exposure control procedures as described in the VNA Infection ControlPolicy and Procedure Manual.

Understandsmedical emergency procedures and the location of the emergency kit(s)/suppliesin all clinic settings. Performs routine emergency kit(s)/supplies checks priorto clinic operation.

Communicateswith other health care providers regarding client continuity of care, afterappropriate consent for release of information provided by the client; andassists in obtaining/completing activities related to requests for medicalrecords.

Notifies clientand initiates follow up on abnormal lab results per applicable protocols.

Maintainsprofessionalism and adheres to client confidentiality in all VNA Health Center practices.

Develops andmaintains a cooperative working relationship with the Clinical Services Managementstaff, other nurses, clinicians, nutritionists, Patient Care Associates(Medical Assistants/Certified Nurse Assistants) and support staff.

Identifies andparticipates in continuing education program/activities including completion ofannual competencies to enhance professional growth and ensure quality of care.

Participates inongoing Quality Improvement activities for the VNA Health Center Clinicprograms.

Nurses may beidentified and given the additional responsibilities to:

Assistin the supervision of the CNA staff and other nursing staff during clinic toensure proper flow, efficient operation and clean up of the clinic.

Assistin the orientation and in-servicing of staff.

Assistin the observation of CNA staff, PRN nurses and other staff nurses for thepurpose of input for job performance evaluation.

Assistin activities needed to help ensure the safety and security of the VNA Health Center clinic clients,staff and facilities.

Monitorlaboratory/mammography results and the initiation of treatment or referral for abnormalsin a timely manner.

Monitorthe follow up of referrals.

Maintainpharmaceutical/vaccine inventory and the supplies needed to dispense; andmaintain the pharmaceutical dispensing logs and vaccine accountability.

Assistin the planning for and scheduling of clinics.

Assistin the development of forms, client educational materials and in thedevelopment or revision of policies, procedures, protocols and standing orders.

Assistin the completion of clinic reports.

Assistin staff and community education; and outreach.

Participatein VNA Health Center committees, as assigned (eg. Education, Infection Control,Forms, Quality Management…).

Assistin the planning and implementation of new programs or services.

Monitorinventory of medical/nursing supplies, pharmaceuticals and client educationalmaterials and order as needed to maintain efficient/cost effective clinicoperation.

• Ability to work allclinic hours as assigned and travel to assigned clinics as needed .
• Able to workeffectively and efficiently in the EHR.
• Completesaccurate and timely documentation for all patient activities. Followsorganization documentation guidelines including electronic and paperprocesses.
• Ensures the patienteducation model of care is fully implemented in their respective areas ofpractice.
• Takes responsibilityfor the implementation of policies and procedures for Joint CommissionAccreditation.
• Familiarizesand complies with all VNA policies and procedures.
• Meetsstandards of behavior expectations.
• Follows established guidelines for use and/or disclosure ofprotected health information. Employees should report any breaches of theHealth Insurance Portability and Accountability Act (HIPAA) rules to thePrivacy Officer immediately. Failure to comply with HIPAA policies andprocedures will result in disciplinary action, up to and includingtermination of employment.
• Other duties as assigned.

Compensation

Hourly Range: $33.00 - $40.00, depending on experience.

Shift Differential

$3.00 for hours worked after 5:00 pm and any hours on a Saturday or Sunday.

Incentives

Earn up to $2,000 annually based on meeting organizational goals.
