Position Summary

The Nurse manager creates, implements and manages the required clinical standards, policies/procedures, protocols, management practices and coordinated quality assurance (QA) activities to ensure the best clinical outcomes for patients/clients. This position works in collaboration with the Senior Director of Clinical Operations to ensure that the Clinical Department goals and objectives are met.

Responsibilities Include but are not limited to:
  • Oversees assigned staff to ensure efficient clinic workflow; streamline workflow processes, scheduling and proper coverage and effectively oversee the management of CCHC’s clinics.
  • Conducts Clinical supervision of LPNs and MAs; ensure quality services are provided to the community and overall compliance with the State of IL Nurse Practice Act.
  • Works in conjunction with the Senior Director of Clinical Operations to effectively monitor clinical operations.
  • Enforces compliance with HIPAA/confidentiality and HITECH regulations.
  • Manages staff in accordance with organization’s policies and applicable regulations. Completes management duties including planning, assigning, and directing work; appraising performance and guiding professional development; rewarding and disciplining employees; addressing employee relations issues and resolving problems.
  • Ensures departmental compliance with all funding sources and applicable city, state and federal laws.
  • Create and implement Policies and Procedures; SOP manual; key performance indicators (KPIs) and Workflows.
  • Ensures that staff has the proper materials, systems access and training to complete job responsibilities, and identifies quality risks and issues and create appropriate corrective action plans to prevent or correct deficiencies in performance of staff.
  • Develops, implements and ensures standardized new employee orientation skills competency training.
  • Collaborates with clinical leadership and completes clinical support tasks & activities, including, but not limited to: phone and in-clinic triage, care coordination/population management, care planning/management, on-site clinical management support at each clinic site, prescription refills, patient hospital/ER follow up, referral and diagnostic tracking within the required standards/guidelines, protocols and policies.
  • Leads monthly meetings/trainings, documents all staff training (ie, sign in sheets, annual training schedule, agenda, handouts) and submits to Human Resources.
  • Implements and monitors MA, RN and LPN student intern/extern program
  • Assists in the development, review and/or modification of clinical protocols, policies, procedures and work flows to ensure compliance with HRSA, CDC, CDPH, NCQA, OSHA, IDPH and other local/state/federal funding source/regulatory bodies’ guidelines.
  • Manages and Coordinates the infection control program across all clinics and programs (ie, medical, dental, etc), including, but not limited to, the following: ensure compliance with federal/state/local clinical standards/guidelines for patient/employee TB tracking, management and reporting; sterilization protocols; hand hygiene tracking and reporting; flu season protocols; implementation of universal precautions; proper disposal of biohazard waste; cleanliness and sanitization of clinic (ie, exam rooms, counters, equipment, supplies, etc.)
  • Address clinical risk management matters/issues in collaboration with clinical management and care team, including patient grievances, incident reporting and identified critical risk assessment and management.
  • Participates in and conducts clinical and administrative huddles. · Oversees compliance with immunizations and life support certification of staff.
  • Responsible for timely inventory, ordering, submission, tracking and reporting of medical supplies, VFC & private stock vaccinations, medications, 1st AID kit, etc.
  • Collaborates with Facilities Operations team on the management, maintenance, testing and reporting of medical equipment.
  • Manages and coordinates the Vaccines for Children (VFC) program, including, but not limited to, the following: oversees, implements, monitors and ensures compliance with requirements; conducts regular (weekly/monthly) internal audits; prepares for external audits; implements/coordinates staff training; ensures proper documentation; completes timely internal/external reporting; and attends VFC external training sessions with identified clinical care team members.
  • Collaborates with the clinical team regarding patient disease management and medication needs.
  • Ensures referral loop process and completion (ie obtaining consult reports, discharge summaries, etc) via collaboration among care team (ie, MA’s, nursing staff; providers, care coordination and Referral teams).
  • Coordinates and tracks ER/ED utilization, hospital admissions, follow up activities/appointments, OB/prenatal reports and ongoing reporting to ensure compliance with FTCA, HRSA, MCO, NCQA/PCMH and other federal/state/local standards and requirements.
  • Participates in meetings/committees, as requested and provides any required status updates and clinical data reporting.
  • Ensures the agency is in good standing with CLIA Waiver and other required clinical certifications; completes renewals as needed. · Develops, coordinates and conducts patient health education and health related materials within the clinics; assists with coordinating community health education/services including mobile health van, community health fairs/events, campaigns, and specialty clinics (ie, flu, diabetes, STD, women’s/men’s health, etc).
  • Conducts team lead activities for identified/assigned clinical projects, including: establishing workplans, leading team meetings, monitoring/evaluating project goals and completing required internal/external reporting.
  • Assists the CMO with provider related matters, including, but not limited to, the following: meetings, trainings, clinical data reporting, EHR orientation/training, and clinical workflows and appropriate documentation.
  • Conducts and participates in random/unannounced and scheduled walk throughs of clinic and program sites to ensure compliance with clinical protocols and standards.
  • Conducts agency-wide clinical related risk management and staff trainings including but not limited to: infection control/needle stick prevention, point-of care, PPE, sterilization, etc.
  • Monitors weekly, and provide monthly reports, and other statistical data for the department as required
  • Works in collaboration with the CMO, Director of QA/Compliance, PCMH core team and care team members to ensure ongoing implementation and compliance with national accreditation standards/regulations and clinical measures including Patient Centered Medical Home (PCMH), HRSA, UDS
  • Participates in Clinical QA process (including PDSA’s, root cause analysis-RCA, etc), record reviews, data tracking/reporting and utilization review (UR) analysis and management.
  • Participates in and provides ongoing electronic health record training for care team
  • Conducts internal audits of 340(b). Submits written results to CMO, CEO and Director, QA and Compliance.
  • Participates on the monthly 340B work group
  • Performs a minimum of 40 quantitative drug transactions (vaccination claims data) reviews per week using the approved self-audit tool. Maintain secured folder of quantitative drug transaction reviews and corrective action plans.
  • Performs other duties as assigned
Minimal Qualifications/ Experience & Skills:
  • BS degree in Nursing. Current unrestricted IL RN License
  • At least 2 years professional nursing experience; preferably in a community health clinic setting
  • Basic Life Support (BLS) certification
  • At least 1 year of experience in working directly with electronic health records (EHR)
  • Familiar with clinical protocols, performance measures and patient health outcomes
  • Experience with community health center and federally qualified health center (FQHC) services/models strongly preferred
  • Must possess strong organizational, administrative and time management skills
  • Must possess strong verbal, computer, and communication skills
  • Prior supervisory experience of Medical Assistants (MA’s) and nurses required
  • Prior administration and management experience
  • Must have vehicle to travel between clinic sites and external meetings.
