Responsibilities

• Conduct comprehensive nutrition assessments and develop individualized care plans.
• Provide medical nutrition therapy to improve patient health and quality of life.
• Manage nutrition support for specialized needs (tube feeding, dialysis, wounds).
• Monitor residents' nutritional status and interventions.
• Collaborate with physicians, nurses, therapists and interdisciplinary teams.
• Provide nutrition education to patients, families and staff.

What You'll Do:

• Conduct comprehensive nutrition assessments and develop individualized nutrition care plans.
• Provide medical nutrition therapy to improve patient health, recovery, and quality of life through the power ofFood as Medicine.
• Manage nutrition support for tube feeding, dialysis, wound healing, weight management, and other specialized nutrition needs.
• Monitor residents' nutritional status and recommend appropriate interventions.
• Collaborate with physicians, nurses, speech therapists, occupational therapists, and interdisciplinary healthcare teams.
• Provide nutrition education to patients, families, and facility staff.
