Job summary

Cobble Hill Health Center in Brooklyn seeks a compassionate Registered Dietitian to assess residents' nutritional needs, develop individualized care plans, and ensure compliance with regulatory standards.

You will collaborate with physicians, nursing staff, and the interdisciplinary team to monitor nutritional status, adjust plans, and drive quality improvement initiatives.

A Bachelor’s degree in Dietetics or related field is required, along with NY licensure and 3–4 years of SNF experience.

• 3–4 years of experience as an RD in SNF.

Responsibilities

• Conduct comprehensive nutritional assessments for residents at admission and regular intervals.
• Develop, implement, and monitor individualized nutrition care plans.
• Collaborate with physicians, nursing staff, and the interdisciplinary team.
• Monitor weight, labs, and dietary intake; provide nutrition education to residents and families.
• Ensure compliance with state, federal, and facility regulations.
• Participate in care plan meetings and quality improvement initiatives.
• Oversee food service operations with dietary staff as needed.

Position Summary

We are seeking a compassionate and detail-oriented Registered Dietitian (RD) to join our team. The ideal candidate will be responsible for assessing residents' nutritional needs, developing individualized care plans, and ensuring compliance with all regulatory standards.

Key Responsibilities

• Conduct comprehensive nutritional assessments for residents upon admission and at regular intervals
• Develop, implement, and monitor individualized nutrition care plans
• Collaborate with physicians, nursing staff, and interdisciplinary team members
• Monitor residents' weight, lab values, and dietary intakeProvide nutrition education and counseling to residents and families
• Ensure compliance with state, federal, and facility regulations
• Participate in care plan meetings and quality improvement initiatives
• Oversee food service operations in collaboration with dietary staff as needed

Qualifications

• Registered Dietitian (RD) credential required
• 3-4 years of Experience as a RD in a SNF required.
• Bachelor's degree in Dietetics, Nutrition, or related field
• Current state licensure (or eligibility) in New York
• Previous experience in long-term care or clinical setting preferred
• Strong communication and organizational skills
• Knowledge of regulatory standards (DOH, CMS)
