Job summary

The Chateau at Brooklyn Rehabilitation and Nursing Center is hiring a Registered Dietitian (RD) to join its clinical nutrition team. The RD will assess nutritional needs and provide counseling for residents in long-term care, short-term, palliative care, and feeding tube patients, collaborating with an interdisciplinary team.

Reporting to the Director of Clinical Nutrition, the successful candidate will apply the Nutrition Care Process, manage duties with minimal supervision, and rotate holidays

Qualifications

• Registered Dietitian (RD) or RD Eligible with verification.
• Comfortable with enteral nutrition.
• Strong oral and written communication skills.
• Familiarity with state survey process and nutrition care regulations a plus.

Responsibilities

• Assess residents' nutritional needs via clinical assessments.
• Provide nutrition counseling and education to residents and families.
• Collaborate with interdisciplinary team to ensure optimal care.
• Apply the Nutrition Care Process in daily practice.
• Demonstrate problem-solving skills and work independently.
• Organize clinical nutrition and patient care responsibilities efficiently.
• Rotate holidays and weekends as required.

Are you a Registered Dietitian (RD) ready to impact the life of patients in a Long Term Care and Rehab facility?

The Chateau at Brooklyn Rehabilitation and Nursing Center is seeking a Registered Dietitian to join their dynamic team. This 189-bed facility is looking for a hard working and energized Registered Dietitian (RD) to help manage the nutrition needs of their short term, long term, Palliative Care and feeding tube residents while proudly serving their diverse community health-care needs.

Registered Dietitian (RD) Reporting to the Director of Clinical Nutrition the successful candidate will:

• Assess the nutritional needs of residents by completing the necessary clinical nutritional assessments
• Providing nutrition counseling and education for residents and family
• Working with interdisciplinary team members to ensure optimal care is provided to all residents
• Be knowledgeable of the Nutrition Care Process
• Possess the ability to creatively problem solve and work independently with minimal supervision
• Efficiently and effectively organize clinical nutrition and patient care responsibilities
• Rotating holidays and weekend work required

Salary: up to $90,000 annually
