Fouraker Hills Rehab & Nursing Center is seeking compassionate Registered Dietitians (RD) to join our team in both full-time and part-time roles within a Skilled Nursing Facility (SNF) / long-term care setting. As an RD, you will conduct nutritional assessments, develop care plans, and provide counseling to residents, families, and staff while collaborating with nursing and dietary teams.

Qualifications

• Licensed RD in good standing.
• Bachelor's degree in Nutrition, Dietetics, or related field.
• Prior SNF or long-term care experience preferred.
• Strong knowledge of clinical nutrition and therapeutic diets.
• Excellent communication, organization, and teamwork skills.
• Resident-centered approach to care.

Responsibilities

• Conduct nutritional assessments for residents in the SNF setting.
• Develop, implement, and update individualized nutrition care plans.
• Provide nutrition counseling and education to residents, families, and staff.
• Collaborate with nursing, therapy, and dietary teams to ensure coordinated care.
• Document assessments, progress notes, and updates according to regulatory standards.
• Ensure compliance with state, federal, and SNF dietary regulations.
• Support food service operations to maintain quality, safety, and resident satisfaction.
• Full-time RD: coverage includes two additional facilities as needed.
• Part-time RD: provide 24 hours per week of dietetic coverage.
