Adelante Healthcare invites a Registered Dietitian to join our Integrated Care Team across West Phoenix, Surprise, Peoria or Goodyear Health Centers. The role offers a Monday–Friday, 8 a.m. – 5 p.m.

schedule with no weekends, and the opportunity to impact patients across the lifespan with evidence-based nutrition plans. You will collaborate with behavioral health, RNs, and PCPs to improve health outcomes, conduct nutrition education, and support self-management goals within Adelante’s

Qualifications

• Four-year college degree in Dietetics or nutrition or related field.
• Two years in community or clinical nutrition education or internship with RD/RDN credentials.
• Current RD/RDN credentials with the Commission on Dietetic Registration.
• Bilingual English/Spanish strongly preferred.
• Travel to multiple locations may be requested.

Responsibilities

• Use the nutrition care process to assess patient nutritional needs and develop care plans in the EHR.
• Provide nutrition education and health promotion to diverse clients.
• Coordinate with care team to maintain accurate patient care plans.
• Educate Health Education staff on nutrition guidelines.
• Refer to interprofessional team members and resources as needed.
• Participate in quality improvement and program evaluation.
