Job summary

SNEB is seeking a Registered Dietitian to perform comprehensive nutrition assessments and deliver medical nutrition therapy as part of an interdisciplinary team.

You will plan individualized care, educate patients and families, and ensure seamless transition of care from hospital to home or other settings. Prior experience as a Dietitian and licensure in the state are required, with registration due within 180 days.

Qualifications

• Bachelor's degree in Dietetics or a closely related field.
• Completion of a Dietetic Internship or Academy of Nutrition and Dietetics approved program.
• Registration as a Dietitian with the Commission on Dietetic Registration within 180 days.
• Licensure as a Dietitian in the state where assigned within 180 days.

Responsibilities

• Performs an accurate assessment of the patient’s nutritional status in a timely manner.
• Plans and implements individualized nutrition care plans with measurable goals.
• Provides medical nutrition therapy to patients and families based on assessment needs.
• Ensures continuity of nutrition care through communication with care providers at admission and discharge.
• Documents all phases of the nutrition care process thoroughly.
