Qualifications

• Registered by the Commission on Dietetic Registration.
• Certification/License in good standing within the state of practice.
• Knowledge of state survey process and compliance with nutrition care regulations (preferred).
• Strong organizational and communication skills.
• Maintain a positive attitude and effectively communicate with occupants, administration, and leadership.
• Compliance with COVID-19 vaccination policies as required by state or customers.
• For multi-site RD, must have a valid Driver's License.

Responsibilities

• Assess clinical nutrition needs of residents/patients and document the plan in the EMR.
• Develop and evaluate individualized care plans based on clinical findings, labs, weight trends, intake, and comorbidities.
• Provide nutrition education and counseling to residents/patients, families, and staff.
• Collaborate with interdisciplinary teams to determine a plan of care that improves health and well-being.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage in multiple buildings based on company needs.
• All other duties as assigned.
