Job summary

Healthcare Services Group (HCSG) is seeking a Registered Dietitian to manage clinical nutrition services at a long-term care facility. You will assess residents\' nutrition needs and develop individualized care plans.

The role emphasizes collaboration with interdisciplinary teams, adherence to CMS and state regulations, and education for residents, families, and staff. A flexible schedule, competitive benefits, and opportunities for professional growth are provided.

Qualifications

• Registered by the Commission on Dietetic Registration.
• State license in good standing.
• Knowledge of the state survey process and compliance with nutrition care regulations.
• Strong organizational and communication skills.
• Compliance with COVID-19 vaccination policies as required.
• RD-Eligible or provisional licensure accepted in certain locations.
• Driver's license if multi-site registration.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and document plans in the EMR.
• Develop and evaluate individualized care plans based on labs, weight trends, and comorbidities.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve health outcomes.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage in multiple buildings as needed and per company needs.
• All other duties as assigned.
