Qualifications
- Registered Dietitian with current CDR registration.
- Active state license in good standing.
- Knowledge of CMS/state nutrition regulations preferred.
- Strong organizational and communication skills.
- COVID-19 vaccination compliance as required.
Responsibilities
- Assess the clinical nutrition needs of residents/patients and document plans in the medical record.
- Develop, implement, and evaluate individualized care plans based on findings.
- Provide nutrition education and counseling to residents, families, and staff.
- Collaborate with interdisciplinary teams to improve health outcomes.
- Ensure adherence to CMS, state, Federal regulations and company standards.
- Provide coverage across multiple buildings as needed.
- All other duties as assigned.
